Evolution Dental Care wants every child to have the opportunity to see a dentist and gain access to care. As part of our efforts we are offering every parent/legal guardian a $5 prepay debit card for bringing their child in for a NEW patient exam, x-rays, prophy, fluoride treatment, and sealants (if recommended).  

To qualify:

– Child must be enrolled in the State of Wisconsin Medicaid program, and be in a HMO we accept.

– Cannot miss or cancel dental appointment at Evolution Dental Care.

– Child must be between the ages of 1-17 years old.

– Child must NOT have been seen at any dental office in the last 12months. 

– Child must cooperate during the appointment.

– Parent/Legal guardian must bring child’s forward health card to appointment.

– Must bring in this promotion letter.  

$5 prepay card is for EACH child that qualifies, with no limitations. A parent can bring in the entire family and receive a prepay debit card for every child that qualifies.

If your child is enrolled in Children’s Community then call forward health (1-800-362-3002), and request to be switched to another HMO.
